When a doctor says your loved one needs skilled nursing, whether that’s wound care, therapy, or clinical monitoring, we connect you with licensed clinical partners. Our caregivers cover everything the clinical visits don’t.
Skilled nursing is clinical care delivered by licensed nurses and therapists, usually in short visits. Noble Health doesn’t provide clinical care. We refer you to licensed home health partners for it, and we fill the hours in between: the help with daily living, the meals, the safety presence that visits can’t cover.
That combination, clinical visits from a licensed partner plus daily support from a Noble Health caregiver, is often exactly what a safe recovery at home looks like.

How it works
- Tell us what’s been ordered. the type of clinical care your doctor has recommended, in general terms.
- We refer you to a clinical partner. a licensed home health agency from our network takes the clinical side.
- We cover the in-between. your Noble Health caregiver handles daily living, so recovery at home is actually safe.
Our partner network
Clinical referrals go to licensed, Medicare-certified home health agencies serving Houston and the surrounding areas. Introductions are based on your needs, never on referral fees.
What Noble Health does not provide
- Live-in care. Noble Health does not provide it and the term is never used.
- Hospice care. Noble Health connects families with licensed hospice partners and supports the household alongside them, but does not provide hospice itself.
- Skilled nursing or any medical service. Referrals go to licensed, Medicare-certified home health partners.
- Medicare or Medicaid billing.
